To register as an organ and tissue donor, you can download a Donor Registration Form HERE. Return the completed and signed form to Second Chance Foundation or the Nevada Donor Network. Your registration form will then be entered into the Nevada Donor Registry.
You can also register as an organ donor at the DMV. However, registering at the DMV does not register you as a tissue donor. You must be registered in the Nevada Donor Registry to be listed as a tissue donor. You can do this by completing a Donor Registration Form.
