Apply for a Grant:

The Patient Services Program offers emotional and physical support to patients and is an important part of our services. Helping patients with daily living expenses, networking them with empathetic and knowledgeable people who understand what patients are experiencing and find ways to make their lives easier are all part of the services we offer.

The Patients' Services Program provides funding for pre and post transplant patients who find themselves in economic short term need due to issues relating to their illness.

Pre and post transplant patients may apply for a grant with the Second Chance Foundation to help them which their economic relief. The grant will be reviewed by a Patients' Services Committee. Funds will be provided on an "as available basis" as to maximize the number of recipients we are able to provide for.

Grants may be limited (but not limited to) for the following necessities:

- Transplant surgical procedure (insufficient or no insurance)
- Immunosuppresant medication (non-insurable portion)
- Transportation (to and from a medical facility)
- Supplemental living expenses
