Become an Organ Donor:

 

The Second Chance Foundation encourages all residents of Nevada to become an organ and/or tissue donor. To register as an organ donor you can do so at the DMV or by registering through the Nevada Donor Network or by completing the form below.

If you wish to register now you can click on the link below, print out the registration form (signature required) and return the registration form to us or to the Nevada Donor Network (Address shown on form). This completed and executed form will be entered into the Nevada Donor Registry. This form offers the option of organ and/or tissue donation. It eliminates the necessity of obtaining consent of your next of kin.

Register Through the DMV or Complete Donor Registry Form?:

 

Nevada’s registered organ donors are identified through a computer database. 
A person can get his/her name in the database by: 
      
      1)       Signing up at the DMV  OR
      2)       Completing a registry form and mailing/faxing it to Nevada Donor Network

 Filling out the registry form does not result in a heart on the driver’s license (as a DMV registration does).  However, the heart designation is merely symbolic, as it is the database and not the actual driver’s license that is used to identify those who wish to donate.

You can purchase "Donate Life" license plates for your car at the DMV.
This one way to show your support for organ donation and provide funding to the
Nevada Organ and Tissue Donation Task Force.
